Place:   Mountain View Golf Club, (27195 SE Kelso Road, Boring, OR 97009)

Check-in: Begins at 7:30 a.m.

Fee: $75 per person ($36 of this fee is tax deductible), please make checks payable to Hood View Junior Academy.

Includes: Greens fees, golf cart, lunch, driving range balls, snacks and prizes!

Payment: Please attach payment and return registration form by Friday, September 11, 2009.
